Transcript Requests

To order an academic transcript from New Haven Unified, please download and complete a Transcript Request Form (click on the link) and submit your request form to transcriptrequest@nhusd.k12.ca.us.

If you need another way to submit your form please contact Ms. Hollie Carcallas at 510-476-2625.

Please allow 5-7 business days for your request to be fulfilled.
